Frequently Asked Questions:
Using PRIME is as easy as activating your PRIME membership, downloading the App, and making your first appointment. If you need help, please call the PRIME team by pressing the orange button.
Next Level PRIME is a membership-based health plan that your employer provides for you and your enrolled dependents. PRIME is completely free for you as the patient unless a small time of service fee has been designated by your employer. Services that may not be free include labs that cannot be run at the Next Level lab, advanced imaging like MRIs and CT scans, and specialist referrals.
Preventive Care & Chronic Care: Yes you do! Let us know you’re coming so we can ensure to have the vaccinations or labs you require on hand.
Urgent Care: You can call ahead or walk right in. However, it is always faster if you let us know you’re coming.
The best way to schedule an appointment is to download the Next Level Urgent Care app on the App store or Google Play store. You can also access the app on desktop by navigating to app.nextlevelurgentcare.com. Once you have logged in, you can schedule appointments for primary care, preventative care, chronic disease management, urgent care, vaccines, health coaching, and emotional wellness coaching. If you are not logged in to the app, you can schedule an appointment to be seen in person at our urgent care locations. If you are unable to log in to the app, you may need to activate your PRIME membership.

Once your Next Level PRIME membership starts, you will be prompted to activate your PRIME membership. All PRIME members, including minors, must be activated prior to their first visit. If you have not received an email link to activate your membership, you can do so HERE. Expect to spend approximately 5 minutes completing activation and be prepared to take a photo of the front and back of your health insurance card(s). If you are unable to activate your PRIME membership or need additional assistance, please call PRIME at 832-957-6200.
With our ‘Skip a Visit’ service, you can conveniently obtain treatments for common conditions like sinusitis, UTIs, pink eye, and stomach bugs, as well as prescriptions for erectile dysfunction, hair loss, birth control, and more, all without the need for an in-person or a virtual visit. You can skip a visit through the mobile app, which can be downloaded HERE.
If you have a health concern and your PCP is unavailable, use PRIME! At the end of your appointment, we will send a summary of your office visit to your primary care physician.
Many common labs, vaccines, and Durable Medical Equipment (DME) are included with your PRIME membership.
Physicals can be performed between 9am – 9pm, 7-days a week. Just download the app and click schedule a visit for an annual physical. You can choose your preferred provider by browsing the provider bio’s on the app. After your virtual visit, your provider will schedule an in-person visit at a clinic that is convenient for you for your exam, blood work, and vaccines
If you are experiencing a medical emergency, please dial 911. For your convenience, we have a medical provider on-call every night for time-sensitive medical concerns. You will be given the option to leave a voicemail that will be handled at 9am or wait to speak to a provider for your urgent medical need.
If we determine that your condition requires a specialist, we will refer you to one IN NETWORK with your insurance. If your condition is emergent, we will send you to the nearest ER.

Please contact your employer or HR department regarding PRIME eligibility, including whether your dependents are eligible for PRIME.
Medical Records, FMLA, & Short Term Disability:
Please email your FMLA or short-term disability paperwork to email@example.com. You will receive a response within one business day. To submit a medical records request, please click here. Our medical records team will reach out to you as soon as your records are prepared. We offer two convenient options for receiving your records: you can have them securely emailed to you, or you can choose to pick up a printed copy at a clinic closest to you. If you need to fax medical records to Next Level PRIME from an outside provider, please use fax number 281-895-3083.
Locations & Hours:
Our clinics are open every day from 9am to 9pm. For a list of locations, or to find a location closest to you, click here. Virtual visits with a licensed medical provider are available every day from 9am to 9pm. For your convenience, there is a medical provider on call from 9pm to 9am every night for urgent needs.
Virtual Visit Access
You will receive a link to your scheduled virtual visit 30 minutes prior to your appointment. Your personalized link will be sent via text and email. Please find a quiet, private location with strong internet connection for your visit.
Outside Imaging & Lab Orders
If you have lab orders or advanced imaging orders written by an outside provider and you would like Next Level PRIME to assist with these orders, please email the orders to firstname.lastname@example.org.
